mental health center forms/sheets?

Brenton Cornwall: Usually these forms are very detailed..they require honesty is u really want to be helped.Some ?s they might ask are about addiction and family history of mental illness or addiction.They may even ask some physical health ?s and require some blood work for hormonal levels and such..If u r referring to just intake papers..they may ask u how u feel on certain days or times of the year..if certain things trigger certain thoughts or feeling. If u have to take an evaluation..they may ask more detailed ?s about more specific thoughts or feelings.. have u take some IQ type exams or common knowledge tests. Bo Perham: Are you refering to a Doctors office or Social Security. Each Doctor will usually have a different form but the basic info is the same. Social Security and Medi care have standardized forms but are so darn complicated, they want to know everything, you can't possibly get it all right just do your best.

Randall Twehous: Yes, most of these kind of places have a general questionnaire to gather basic information on your psychical and mental state. Some questions may sound strange, but do yourself a favor, and answer as honestly and sincerely on how you answer those questions. The forms will include, moods, any changes in your life, your nutrition and how your are eating, any meds you ! are on, why you came to the center, or what do you hope to acc! omplish at this center, any strange behavior, your relationship status, your age, gender, amount of education, are your parents together, who do you live with, what kind of job you have, how you get along with friends, family, and co-workers, etc., etc... Answer as best you can, don't over do it, but also make sure that you include any rough times, feelings, moods, etc. Just be honest, The questionnaire is just giving the health care person some basic information on you, to get things started.
